UK companies generally need to file a confirmation statement at least once every 12 months, based on the company's own review period rather than your accounting period. This is separate from the annual accounts deadline. A company can also file more than one confirmation statement during a review period when its information changes, which is useful if you want the public record updated sooner rather than later. The review period dates belong to the company, so it is worth checking when yours falls due.

The confirmation statement is a check that the Companies House record is correct. Some changes cannot simply be confirmed on the confirmation statement itself and need to be reported to Companies House separately, often as a different filing made at the time the change happens. Yes, in most cases. A dormant company or a company that is not trading still has a record at Companies House, and that record still needs to be confirmed. Dormancy affects your accounts and your tax position rather than the confirmation statement requirement. Many owners assume that because there has been no trading there is nothing to file, which is a common cause of late confirmation statements and an out-of-date record. It is worth confirming your position if you are unsure.

They are three separate filings with different purposes. The confirmation statement confirms that the information Companies House holds about the company is accurate and up to date. Annual accounts report the company's financial position and performance for the accounting period and go to Companies House (and to HMRC where relevant).
