The requirement normally applies whether the company is actively trading, dormant or not trading at the moment, and it applies to single-director and single-shareholder companies too. If you are unsure whether your company needs to file, it is worth checking the company's status and its current review period at Companies House before assuming you are outside the requirement.
A company generally needs to file at least one confirmation statement every 12 months, based on its review period. The review period normally starts on incorporation or on the date of the last confirmation statement. This 12-month cycle is not the same as the company's accounting period for annual accounts, nor is it the deadline for a Corporation Tax Return, so it is worth tracking separately. If your company's information changes during a review period, you can file more than one confirmation statement so the register reflects the position sooner rather than waiting for the next annual one.

In most cases, yes. Dormant companies and companies that are not currently trading generally still need to file a confirmation statement at least once every 12 months. Dormancy affects what you report in your annual accounts and your tax position, but it does not normally remove the confirmation statement requirement, because the purpose of the filing is to keep the public register accurate rather than to report financial activity. For a company that has not traded at all during the period, the confirmation statement is often a short and straightforward filing.
These are three separate filings with three different purposes. A confirmation statement confirms that the information Companies House holds about the company is accurate and up to date. A Corporation Tax Return (form CT600) is submitted to HMRC to report the company's taxable profits and is not filed through the confirmation statement.
