The exact deadline depends on your company's accounting reference period, whether it is your first set of accounts and your particular circumstances, so it is worth checking your company's own position on the Companies House record rather than relying on a general assumption.

There are many reasons. A director may not have realised that a filing deadline had passed, trading may have stopped partway through a period, bookkeeping records may be incomplete, or the company may have changed accountants or been dormant for a time. Sometimes there are several periods outstanding because an earlier one was never completed. Whatever the reason, the practical position is the same: the required accounts still need to be prepared and delivered, and it is usually easier to deal with the matter once the full picture is clear.

In many cases, yes. Dormancy and overdue filing are separate issues: a dormant company can still have an outstanding filing obligation if the required accounts were not delivered by the applicable deadline. Dormant companies are not automatically exempt from annual accounts filing, although the accounts they need may be simpler than those of a trading company.
